Forward the following documents to the Commissioners' Club office at Room 808 Hong Kong Scout Centre: Duly completed Membership Application Form (Original) Membership Visa Credit Card Form of Wing Lung Bank Hong Kong Identity Card (Copy) Photo copy of valid appointment or Warrant issued by Scout Association of Hong Kong or the Hong Kong Girl Guides Association If applicable, certificate of the Award(s) or Wood Badge conferred by Scout Association of Hong Kong or the Hong Kong Girl Guides Association.(if have) Proof of income (Copy) Proof of residential address, e.g. utilities bills (Copy) For application for a Membership Visa Supplementary Credit Card(s), please complete relevant parts of the Membership VISA Card Application Form and supply the following documents: Membership Visa Supplementary Credit Card(s) Applicant's Hong Kong Identity Card (copy) Proof of relationship (Copy)
